Documentation to

Be Submitted:

　　a. Completely filled in VISA OR RESIDENT PERMIT APPLICATION FORM, two recent 2-inch half-length, bareheaded and full-faced photos;

b. Data page of valid passport, and copy of the visa with the latest entry stamp of the applicant (original required) ;

c. The original of the certificate of check-in in Ningbo (issued by the hotel or the police substation of the area where the lodging takes place)

d. the birth certificate of the applicant and duplicates of passports of parents. And if one of the parents is Chinese, the duplicate of the overseas certification of permanent domicile should be submitted.

e. the authorization by the applicant’s parents stating the entrusting, the guardian entrusted and the period of foster;

f. the identification of the foster or the guardian

g. the notarized foster care guarantee

h. Other credentials required by the entry and exit departments of the public security bureau
